TERMS AND CONDITIONS OF ENTRY

Method of entry
|
Readers enter the competition through two methods. (1) Sending in a completed entry form published in Australian Doctor. Entry coupons will appear in each issue of Australian Doctor for a total of 8 issues. (2) Online through www.australiandoctor.com.au. The online entries will require GPs to log onto the website, put in their details (name, address, surgery address, telephone number, QA number) and submit an entry form. Once a GP has completed the online entry form, they click submit and the information goes into two places: (1) the Australian Doctor database and (2) an email sent out to Reed Business Competition organiser: Caroline McLean, Australian Doctor Marketing. The winner will be drawn manually. To get the electronic entries from the website into a physical entry will require an emailed message into the competition organiser’s inbox for each entry. Once the entries are in the inbox, each entry will be printed once and put into the barrel with the other coupon entries from the Australian Doctor magazine.
Each person can only enter once through the online submission form. There will be a lock that is placed onto the amount of times a person can enter into the competition, if they have already entered. The online entries will be for registers online.

The competition will commence on Monday 3 October 2005 and will close on Monday 5 December 2005.

Draw details
|
The draw will be conducted at the offices of Reed Business Information Pty Ltd, Tower 2, 475 Victoria Avenue, Chatswood NSW 2067 on Wednesday 7 December 2005 at midday. All entries, including the printed online entries will be placed into a barrel and the first entry drawn will win.

Notification of winner
|
The winner will be notified by telephone, mail and published on www.australiandoctor.com.au. The winner’s name, address and a profile will be published in Australian Doctor on Friday 16 December 2005.

Unclaimed prize
|
If the prize is not claimed within 4 months of the draw date, an unclaimed prize draw will be conducted at Australian Doctor, Reed Business Information, Tower 2, 475 Victoria Avenue, Chatswood NSW 2067 on Monday 10 April 2006 at 12 noon subject to any written directions given under the Trade Promotions regulations of New South Wales.

Entries
|
192,000 entries in total will be printed. Approximately 6000 entries will be collected (8 issues of Australian Doctor at 500 entries per issue) and approximately 12,000 doctors are registered to enter online entries through www.australiandoctor.com.au, expecting approximately 1000 entries. These are distributed Australia wide and all registered Australian Doctor personnel can go enter online once, due to a lock that has been placed.

Terms and Conditions
- Entry is only open to Australian residents who are practicing GPs. Australian Doctor employees (and their families) plus contributors are not eligible to enter.
- Only one entry per person will be accepted. Faxed or photocopied entries will not be accepted.
- Online entries can be made through www.australiandoctor.com.au under Fujitsu Competition on the homepage. All entries online must be registered GPs and fill out the mandatory forms and areas. Once an online form has been submitted a lock is placed to ensure only one entry per person can be made online for this competition.
- Promotion commences Monday 3 October 2005. Entries close Monday 5 December 2005 at 5pm. The draw will take place at midday, Wednesday 7 December 2005, at Reed Business Information, 475 Victoria Avenue, Chatswood NSW 2067. The winner will be notified by mail and published in Australian Doctor on Friday 16 December 2005. If the prize is not claimed within 4 months of the draw date, an unclaimed prize draw will be conducted at Reed Business Information on Monday 10 April 2006 at noon subject to any written directions given under the Trade Promotions regulations of each state of Australia. The judge’s decision is final and no correspondence will be entered into.
- The prize includes one Fujitsu P42VHA4OWS Plasma Screen. No part is refundable or redeemable for cash.
- Total prize value is $7999, but this may change depending on place of freight. The prize excludes all domestic taxes and insurances. The Promoter accepts no responsibility for any variation in the value of the prize.
- The Promoter shall not be liable for any loss or damage whatsoever which is suffered (including but not limited to indirect or consequential loss) or for personal injury which is suffered or sustained during the course of the prize except for any liability which cannot be excluded by law.
- The Promoter accepts no responsibility for late, lost or misdirected mail.
- All entries become the property of the Promoter.
